| Description (Service) | Provides culturally sensitive homemaking and assisted living care services to older adults living in Midland, Wasaga Beach and Collingwood Services include: * light housekeeping * laundry * sweeping/mopping and dusting * personal hygiene care * medication support * meal preparation * telephone reassurance * navigation of community services Program is a partnership between March of Dimes Canada and Barrie Area Native Advisory Circle |

| Eligibility | * Individuals must self-identify as First Nation, Metis or Inuit * Individual must be an older adult (55+) and/or an older adult with age related health conditions or disabilities * Individual resides within Midland, Wasaga Beach or Collingwood (North Simcoe and South Georgian Bay) |
| Application | * Call 705-818-3080 to speak to Indigenous Independent Living Care worker (IILC) * referral form can be filed out through self-referral, partnering agency, community organization or from a family physician * form can then be directly submitted to the IILC worker to confirm the eligibility * IILC worker will personally meet with the individual that is new to the program, to fill out the intake and assessment forms to identify any needs or requirements for the services provided, to develop a plan of care and to bridge the new connection between client and IILC worker. |
