Description (Service) Medical clinic focused on family and community health * nurse practitioners work with collaborating physicians, registered nurses, social workers, registered dietitians and pharmacists to assess and treat non-life threatening injuries or illnesses 
Services available:  
* annual physicals (male and female) 
* minor injury/illness 
* immunizations 
* smoking cessation 
* counseling  
* nutrition education 
* well baby/prenatal 
* medication management 
* monitoring and management of chronic diseases such as diabetes, arthritis, asthma, and heart disease 
* health promotion 
* fall prevention program for older persons

Fri * 8:30 am-3 pm

Eligibility Resident of Simcoe County * must not be on a roster with a family physician
