Description (Service) Nurse Practitioners work with collaborating Physicians, Registered Nurses, Social Workers, Registered Dietitians and Pharmacists to assess and treat non-life threatening injuries or illnesses, services include:
  • annual physicals
  • minor injury/illness
  • immunizations
  • smoking cessation
  • counseling
  • nutrition education
  • well baby/prenatal
  • medication management
  • monitoring and management of chronic diseases such as diabetes, arthritis, asthma, and heart disease
  • health promotion
  • fall prevention program for older persons

Hours Mon,Wed,Thu 8:30 am-4 pm * Tue 8:30 am-6 pm * Fri 8:30 am-3 pm * subject to change * in cases of inclement weather, please call the clinic
Areas Served Simcoe County
Eligibility Residents of Simcoe County and must not be on a roster with a family physician
