Description (Service)A community mental health and primary care centre that promotes and enhances mental, emotional, and physical well-being through education, collaboration, advocacy, and the provision of a broad range of direct services.  
Programs offered include:  
* Community Access Services - first point of contact for the community 
* Nurse Practitioner-Led Clinic with onsite Pharmacy 
* Community Wellness Services 
* Housing Case Management 
* Transitional Rehabilitation Housing Project (TRHP) 
* Assertive Community Treatment (ACT) 
* Community Treatment Order (CTO) 
* Health Promotion, Public Relations - education and training services 
* Youth After School Program (YASP)  
* Involved in partnership initiatives throughout Durham Region with other service providers
